Client Intake Forms

Welcome to the New Client Center for Vitality Natural Health and Wellness Center! Upon entering our office, our front desk staff will welcome you as a member of our family. We look forward to working with you in your health and wellness journey.

Once you have scheduled your initial appointment, you will receive two emails. The first email will confirm your appointment type, date and time. The second email will include the details of your appointment as well as a client intake form to complete for the type of service scheduled. This intake form outlines your current health concerns and asks questions about your health history and profile. It is recommended that you download and print the intake forms and complete prior to your appointment as it could take 30 minutes or longer to complete for some services. If you are unable to complete the form prior to your appointment, please arrive at least 15-30 minutes before your scheduled appointment to complete the necessary forms. Please note that you will also receive the following reminders before your appointment:

  • An email remnider 2 days before your scheduled appointment
  • A text reminder 24 hours before your scheduled appointment (if you have a cell phone on file)
  • A phone call reminder 24-48 hours from our staff before your scheduled appointment.
